Appointments Policy
New patients and returning patients not seen for a year are asked to fill out a health history questionnaire. If there is sufficient time when you call to schedule your appointment, this will be mailed to you; you may also download the form from this website. Please fill it out just prior to your appointment, and bring the completed form to your appointment. Please arrive 15 minutes prior to your scheduled appointment so this information can be reviewed and processed. ACCURATE, COMPLETE AND UP-TO-DATE INFORMATION IS VITAL TO US IN PROVIDING YOU THE EXCELLENT CARE YOU DESERVE. We will ask you to fill out any missing information at the time of your appointment, which may cause delays please come prepared.
Returning patients (those who have been seen at OSENTA within one calendar year) need not arrive prior to their scheduled appointment time.
It is our policy to attempt to contact you the afternoon prior to your appointment as a reminder. We will leave a message for you if an answering machine or voice mail option is available. If you prefer for us to not leave a message, please inform us at the time that your appointment is scheduled.
Cancellations/Rescheduling
We understand that patients will cancel or need to reschedule appointments. If you need to reschedule or wish to cancel an appointment, please have the courtesy to let us know at least 24 hours before the appointment so that we can make the time available to other patients. We will not charge you for the first occurrence of a missed appointment. However, if two appointments are missed you may be charged $25.00 for a third missed appointment.
Children/Minors Policy
Patients under 18 years of age are required to be accompanied to their appointments by an adult. Decisions to proceed with surgery or other procedures must be made by the patients legal guardian, who would sign relevant consent forms for treatment.
Billing Policy
Patients are expected to pay for services at the time they are rendered. We will provide you a written statement, explaining all charges. Cash or personal check are accepted forms of payment. If you have insurance, you are responsible for knowing what benefits your insurance policy provides you, including amounts of co-payments, co-insurance, and covered services. We understand these can be confusing issues and our staff will be friendly and tireless in trying to help you, but remember, it is your responsibility to know about your insurance, so please be patient and pleasant, and help us to help you.
Discharging Patients from OSENTA
Please be aware that we at OSENTA view our role as one of resource, advisor and supporter with regards to your health. There will be times when patients will not follow our health recommendations, such as forgetting or disregarding care recommendations, not filling prescriptions, repeatedly missing appointments. While we attempt to be understanding, and always grant that you are in charge of your own body and health decisions, if such behaviors are repetitive and felt to be detrimental to your health it may be necessary to discharge you from our care, and ask you to transfer your care to another provider. We may also need to discharge patients who exhibit abusive, inappropriate or dangerous behavior towards our staff members or office, or who have unpaid accounts and have not attempted to make arrangements with our billing specialist. Please rest assured that prior to such drastic measures, we will make every effort to discuss these issues with you, and to reconcile such problems before discharging you from our care.
